13 year old cat with thinning coat and skin problem

My 13 year old American Shorthair has a coat that's getting thinner every day. He also has a skin problem with scratching to the point of scabbing under the neck and front shoulders. What can I do?
I've used everything from Vitamin A and D, to flea controllers to homeopathic and nothing seems to work.
